      Welcome to GNH Community, glad to have you join. You are now able to post information that you think will be of interest to other nonprofits, civic organizations, the people they serve or the general public. You may post calendared events on the events page, non-date specific information on the blog, pictures or video.

       

      Check out the groups created by other members; this is a great place to meet people that share your interest and to ask specific questions. You may request to join as many groups as you wish or start a new group. The creator of a group sets the parameters for membership. If you have a question about a group, please write to the owner (the person who initiated the group). If you start a group you can make it open to anyone or limit membership.

       

      If you have questions or suggestions as to how we can make GNH Community more useful for you please let me know.

    Description Interest

    Project Access-New Haven (PA-NH) is a 501(c)(3) nonprofit organization founded in 2009 by a group of dedicated physicians who sought to address health concerns and inequities identified within the New Haven community. PA-NH, which is based on a program model that has been implemented in more than 50 communities across the U.S., increases access to specialty care and ancillary services for low-income, uninsured individuals in the Greater New Haven area by coordinating the provision of donated medical care and services and ensuring the delivery of comprehensive care to this population. PA-NH provides care through a network of over 300 volunteer physicians who donate their time and Yale-New Haven Hospital (YNHH) and the Hospital of Saint Raphael (HSR), which donate inpatient and outpatient medical services. At the core of the program are Patient Navigators who help patients obtain needed services and navigate a complex, confusing, and often intimidating healthcare system. By providing access to comprehensive care in a timely manner, PA-NH also reduces health disparities and fosters cost savings for the local healthcare system.
